Health Requirements for District 205 Schools
Kindergarten:
Health Exam with Immunization Record
Vision Exam by Ophthalmologist or Optometrist
Dental Exam
2nd Grade:
Dental Exam
6th Grade:
Health Exam with Immunization Record
Dental Exam
9th Grade:
Health Exam with Immunization Record
Health Exams must be turned in prior to the first day of school.
Students will not be allowed to attend school without a health exam.
Vision Exam due October 15th.
Dental may be turned in during the school year.
